Description

NHS Hounslow Clinical Commissioning Group (HCCG) is inviting suitably qualified and experienced Providers to register their interest in this future opportunity. The purpose of this engagement is to inform the commissioning approach for the future Hounslow integrated consultant led community pulmonary rehabilitation, home oxygen assessment and specialist nurse support service and to gauge the level of market interest.
The overall aim of the Hounslow integrated consultant led community pulmonary rehabilitation, home oxygen assessment and specialist nurse support service is to provide all eligible patients with a service that meets their needs and achieves the service provision outcomes:
1. Reduction in the number of A&E attendances and Non-Elective admission (NEL) for patients with chronic respiratory conditions e.g. COPD
2. Improved health outcomes for patients with chronic respiratory conditions e.g. COPD i.e. fewer exacerbation, less absence through illness and less hospitalisation
3. Improved patient awareness and self-management of their own chronic respiratory conditions e.g. COPD condition, i.e. more independent management of their own condition.
4. Improved patient satisfaction of the integrated consultant led community Pulmonary rehabilitation, home oxygen assessment and specialist nurse support service
5. Improved GP awareness of chronic respiratory conditions, diagnosis and treatment
6. Integration with other services i.e. hospitals, social services, voluntary sector, health and social care, other community services and mental health services.

Should NHS Hounslow CCG opt to proceed with a competitive procurement process for the delivery of this service, it is anticipated that a contract will be awarded for 3 years to the appointed provider with the option to extend for up to a further 2 years at their discretion. The expected service commencement date is anticipated to be 1st April 2018. The value of the contract is estimated at £1,820,000 over 5 years. ***INTERESTED PARTIES ARE ASKED TO RETURN THE ATTACHED QUESTIONNAIRE TO: ekkehard_kugler@nhs.net REFERENCING ' COMMUNITY PULMONARY REHABILITATION' IN THE TITLE. If a formal procurement process is to take place, HCCG anticipates publishing a Contract Notice to commence procurement during late summer 2017. The submission of an expression of interest is not a prerequisite to any subsequent procurement application; however the response to this market engagement advert will help to inform Hounslow CCG's commissioning approach, including whether a competitive process will be appropriate, i.e. a lack of expressions of interest may be used to evidence the appropriateness to award a contract direct without a competition.
