Description

Camden Council is planning to re-commission Minding the Gap. Minding the Gap (MTG) is a programme designed to improve mental health of young people aged between 16 and 25 years of age in Camden. It specifically aims to bridge the gap between children's mental health and adult mental health services. The programme's key components include interventions to:
• Create a more straightforward, responsive and easy access referral pathway
• Decreased the number of patients who disengage in the transition from child services to adult services
• Develop systems to identify, target and engage those most at risk
• Improve early intervention and prevention to reduce the number of young people seeking help at crisis point
• Deliver client friendly services
• Provide young people with the opportunity to co-design the service delivered in their borough
