Description

One Croydon Alliance Programme Management Office is inviting expressions of interest from suitably experienced and capable providers, to provide a coordinating and facilitating role to enable the recruitment, development and oversight of a brand new team of Care Home Coordinators who will work across the London borough of Croydon.

Funding of up to £285,000 has been secured to run this new service as a pilot for at least 12 months, with the aim for the new Coordinators to act as the link between "Integrated Community Networks Plus" (ICN+)* Locality Teams and local care homes, joining up care across the system in Croydon.
This new team of Coordinators will also provide care plans for people in the community predominantly for people over the age of 65.

The coordinating provider will be specifically invited to employ and manage 6 (six) ICN+ Care Coordinators during this pilot phase.

*Note: the ICN+ is a major programme of transformation and integration that will improve outcomes for Croydon people through a proactive and preventative approach within each of the six Localities of the Borough. It focuses on all adults and is aligned with services for children and families. The rollout of the ICN+ integrated community teams has already started to see positive outcomes for Croydon residents. The new ICN+ Care Home Coordinator role is being introduced to further enable the model to be sustainable and to proactively support residents to stay well.

Current commissioning intentions assume that a contract for the new pilot service can be awarded during May 2022 to enable recruitment and ramp-up of the service during the summer of 2022. The expected budgetary allocation for this pilot service will be up to £285,000 for 12 months.
