Description

Cornwall Council and NHS Kernow Clinical Commissioning Group have been working in partnership with stakeholders in Cornwall to redesign the way Home Care and Supported Living Services are contracted and delivered in Cornwall with the aim to tendering these services with a contract start date of June 2018.

These services support people to live at home, engage with their local communities and build a person's confidence, skills and strengths enabling people to maximise their independence, enhancing their health and wellbeing and achieving positive outcomes.
