Description

North Kirklees Clinical Commissioning Groups (CCGs) are in the process of re-procuring their community based respiratory services for people living in the North Kirklees boundary. In line with the NHS Five Year Forward View and transformation the CCGs is very keen to maximise the use of community services within their service delivery offer to improve access for patients, and help manage demand effectively and efficiently.

This is a Market Testing exercise relating to the provision of community respiratory services which will, where possible provide an alternative to hospital based services for people with respiratory conditions, for those people residing in a Residential / Care home.

We are looking for a service that will offer a holistic approach to the following (further information can be found under attachments):

1) Admission Avoidance
2) Early Supported Discharge
3) Assessment & Review Home Oxygen Service (HOS). *Please note there may be scope in the future for the footprint of this HOS-AR to be reviewed and expanded
4) Pulmonary Rehabilitation Service
North Kirklees CCGs are currently exploring the merits of commissioning an Accountable Care Organisation (ACO) model to provide all health care into our care homes with a timeline of an integrated approach by 2020. It is possible that any contract awarded to a suitable provider between now and then may transfer across to the ACO.

The current intention is for the initial contract to be for up to 2 years, with an option to extend the contract at the Commissioners discretion for up to 2 years. The value of the contract is anticipated to be circa £410,000 per annum.
