Description

Blackpool CCG Community Pulmonary Rehabilitation tender opportunity

NHS Midlands and Lancashire Commissioning Support Unit (CSU) are working on behalf of NHS Blackpool Clinical Commissioning Group (CCG) who wish to procure a Community Pulmonary Rehabilitation service for patients living in the town.

The Community Pulmonary Rehabilitation Service will be awarded to a single provider with the intention for a hybrid model of delivery which compromises traditional face-to-face services within a community setting, but also encourages wider participation in a web based self-management application (provided to the potential supplier by NHS Blackpool CCG). The application will be funded by the CCG and the cost has not been incorporated into the affordability of the tender.

The service will provide a multidisciplinary programme of care for patients with chronic respiratory impairment which is individually tailored and designed to optimise each person's physical and social performance and autonomy.

The successful supplier will accept referrals from General Medical Practitioners, Practice Nurses, Integrated Neighbourhood Teams, Respiratory Consultants, Respiratory Nurse Specialists, the Rapid Response Team and as reasonably directed by Blackpool CCG's Contract Manager. The Contract shall be for a minimum period of three years with options to extend for 18 months.
