Description
NHS Brent Clinical Commissioning Group (CCG) is commissioning an Early Supported Discharge service for Stroke patients registered with a Brent GP practice.
The new service will comprise an Early Supported Discharge service for stroke patients, medium-term community stroke rehabilitation and education and training for primary care practitioners. Its central objectives are to provide high quality care in patients' homes enabling them to be rehabilitated faster in the community.
The objectives of the new service are:
• Improve the experience and health outcomes of patients who have had a stroke in Brent
• Reduce the length of stay throughout the patient care pathway, particularly in the Acute Stroke and Community Rehab Units
• Reduce the number of readmissions for stroke patients within 30 days of discharge from hospital/rehab units
• Increase the number of newly diagnosed stroke patients receiving care in their own home after discharge from hospital
• Increase the rehabilitation and recovery rates from onset of condition
• Enable stroke patients to become more confident and informed health service users
• Improve primary care awareness of the needs of stroke patients
The CCG is offering a contract to provide the service for a three year period commencing from 1st April 2017 with the option of a further two year extension.

Procurement Process
The Services being procured fall within those health care services to which the NHS (Procurement, Patient Choice and Competition) Regulations 2013 (No.2) relate. As such, the Services to which this Procurement relates fall within Schedule 3 of the Public Contracts Regulations 2015 (the "Regulations").
The procurement of the Services is being run as a bespoke, single-stage application process akin to the Open Procedure.
No terms used through this Procurement or any other indication should be interpreted to mean that the Contracting Authority intends to hold itself bound by any of the Regulations, save those applicable to Schedule 3 services.
