Description
NHS Arden and Greater East Midlands Commissioning Support Unit (AGCSU), on behalf of NHS England North Midlands (the Commissioner), is inviting suitably qualified and experienced providers to deliver a Primary Care Occupational Health Service (OHS) in Derbyshire.
The service is expected to be fully operational, providing services from 1st December 2018. The NHS England standard terms will apply to any contract arising from this quotation.
The Occupational Health Service will have three distinct elements:
1. That which will be funded by NHS England including a core service for performers registered on the national performers list
2. That which will be self-funded by applicants to the national performers list
3. A service made available to independent primary care contractors in support of their employer responsibility (funded by Primary Care employers)
This contract will cover provision of the Occupational Health Service to Primary Care in Derbyshire and will be for a period of 4 years with the options to extend for an additional 9 months.

Contract Value:
Based on the current contract it is anticipated that the service provision resultant of this procurement will have a total contract value for the lifetime of the contract of approximately £33,000 (excluding VAT).

Your response must be valid for acceptance for 90 days from the deadline for receipt of proposals. Your response constitutes an offer and if the Commissioner accepts that offer then a legally binding contract will exist between NHS England North Midlands and your organisation.
Respondents accept that the Commissioner is subject to the Freedom of Information Act and government transparency obligations which may require the Commissioner to disclose information received from you, to third parties.
This letter and your response do not give rise to any contractual obligation or liability unless and until such time as the Commissioner issues a letter referencing this Request for Quotation accepting your proposal. The Commissioner does not make any commitment to purchase and shall have no liability for your costs in responding to this Request for Quotation.
