Description

The London Borough of Merton together with Merton Clinical Commissioning Group wish to commission an integrated health and care services model from April 2021 to ensure long term sustainability. This work is a key priority within the Merton Health and Care Plan and for the Merton Health and Care Together Programme.

We wish to commission differently to secure community services which are person centred, support prevention and are fully integrated across physical health, mental health and social care to deliver better outcomes for our residents.

A four-week engagement window will begin on 13th January closing on 10th February.

On January 13th, the CCG together with the London Borough of Merton will publish 'Engagement Document' which outlines our collective vision and outcomes for community services in Merton. NB: A COPY OF THIS DOCUMENT WILL BE UPLOADED TO THIS NOTICE AND BE AVAILABLE FROM MONDAY 13TH JANUARY

In addition, a provider engagement event will be held on 16th January at Merton College in which you are invited to participate. No preparation is required for the event.

Participation in the engagement exercise is not a mandatory requirement for participating in any potential future procurement; however, responses received will assist to inform the Commissioners as to the level of interest from the market and will be used to inform the procurement approach.
