'Singing for the Brain' music activity group sessions for people with dementia and carers delivered weekly, alternating between virtual sessions on Zoom and face to face sessions at community venues.

A new contract is put in place for a period of 18 months to afford the newly forming NHS North West London CCG time to agree the approach to commission this service at NWL ICS level in line with national and ICS guidance to ensure smoother, less complicated, non-duplicated service delivery.

• Improving access to healthcare,
• Working in partnership with both primary care and Hospitals to improve access to the range of pathways for ill health prevention,
• Health promotion and community treatment options.
• When a patient requires hospitalisation the projects support efficient and effective safe discharge protocols that ensure smooth transition back in to the community and reducing the likelihood of readmission.
• Ensuring there is a strong VSC offer strengthens our local community

It is anticipated that this not only improves health outcomes for this vulnerable group but may also reduce costs incurred by avoiding extended hospital stays and improving patient flows.
