Description
City of Wolverhampton Council wish to establish a co-ordinated and comprehensive post dementia diagnosis support service that comprises of three components but operates as a single entity with a single point of contact. The service will comprises of three components: Dementia Navigators (DN), In-Reach Community Support, and Provider-Led Information and Educational community sessions. The DN service is available for all Wolverhampton eligible residents and their carers. The In-Reach Community Support and Community Sessions builds upon the DN element to provides a support network for eligible residents and their carers.
