Description
The City of Wolverhampton is commissioning a comprehensive post dementia diagnosis support service. The service comprises of two components: Dementia Navigators (DN) and Community Café. The DN support service is available for all Wolverhampton residents with a dementia diagnosis and their carers. The community café builds upon the DN element and provides a support network for eligible residents and their carers; with service users, carers and their families actively encouraged to contribute to, and support the operation of the café.
