Description

Nottingham City CCG invites suitably qualified and experienced providers to deliver community podiatry services under a lead provider model incorporating core podiatry, domiciliary, high level biomechanics, specialist podiatry and podiatric surgery.

Aims of the Service:
The aim of the service is to provide an integrated podiatry service in the community for patients with more urgent and routine podiatry needs. In particular, the following will be included within the service;
• single point of access and clinical triage,
• urgent referral into hospital based MDT clinic within 24 hours,
• provision of a specialist podiatry service to see patients within 5 working days to prevent further deterioration and offer more regular follow up,
• provision of a routine podiatry service seeing patients within 30 working days
• provision of community-based podiatric surgery with on-site x-ray facilities with a 12 -week referral to treatment target
• education and training to GP practices regarding annual diabetic foot checks

The procurement opportunity also offers the option of a social nail cutting service. This will not form part of the commissioned pathway and will not be a pre-requisite to a tender response but the CCG would like to encourage a self (patient)-funded finger and toenail cutting service as added value to the pathway.

The successful provider is asked to deliver the full pathway or sub contract elements of the service under a lead provider model.

Outcomes that the service will deliver include:
• Patients will receive care based on their individual needs
• patients will experience improved foot health, mobility and independence
• patients will experience reduced foot pain
• patients will say they definitely understood an explanation of the treatment plan and were aware of self-help strategies
• patients/carers will rate the overall service experience good or excellent
• patients will say that they were always treated with dignity & respect
• patients will be satisfied with waiting time
• it is anticipated that referrals and activity within Acute MDT clinic will be reduced based on increased specialism and access within community service.

The contract will be available for a 3 year period with the option to extend for up to 2 further years.

Following initial scoring of the tender (as set out in the evaluation questionnaire) successful Providers having passed the first stage will be invited for a Clarification Interview which will be held week commencing 21 December 2015 at the CCGs Head Quarters in Nottingham.
