Description

North of England Commissioning Support Unit (NECS) working for and on behalf NHS Cumbria Clinical Commissioning Group (CCG) who wish to commission a Care Home Scheme in the Carlisle locality.
The Carlisle Care Home Scheme is a mobile community service comprising a multi-disciplinary of a General Practitioner, Senior Nurse, Pharmacist, Registered General Nurses and Support Staff that works in close partnership with the care homes and residential homes within the Carlisle locality. The service aims to improve the quality of care received for Carlisle residents and to enable the patient and their family to be central to the decisions made about their own care, aiming to reduce inappropriate journeys to Accident and Emergency and unnecessary admissions that are unlikely to benefit the individual. Collaborations for the Carlisle Care Home Scheme are encouraged.
The contract term for this service is 1 year with the option to extend for a further 12 month period.
A bidder event will be held on Thursday 17 December 2015 between 14.00pm and 15:00pm at Cumbria CCG Carlisle Locality Office, Rosehill Industrial Estate, 4 Wavell Drive, Carlisle, Cumbria, CA1 2SE.
