Description

The Commissioners are seeking suitably qualified providers to deliver a Mild to Moderate Hearing Loss Service for Adults Aged 55 and Over (18+ for Solihull). This service is being commissioned using an Any Qualified Provider process.

The qualification period for the AQP accreditation will be for 3 years from the 1st July 2016.

This AQP is being undertaken on behalf of:
• Birmingham Cross City CCG
• Birmingham South Central CCG
• Sandwell & West Birmingham CCG
• Solihull CCG
• Wolverhampton CCG
• Walsall CCG
• Dudley CCG

Aims and Objectives of Service

The aim is to provide a comprehensive patient-centred direct access adult hearing service for age related hearing loss for adults aged 55 and over (18+ Solihull CCG) in line with national guidance and local requirements.

Service Description

The Service will deliver a community based, adult hearing assessment service, including hearing aid fitting (where required), follow-up and aftercare services for adults aged 55 or over (18+ Solihull CCG), with suspected or diagnosed age related hearing loss who have been referred by their GP.

The service required is for a community based, adult hearing assessment service, including hearing aid fitting (where required), follow-up and aftercare services for adults aged 55 or over (18+ in Solihull), with suspected or diagnosed age related hearing loss for the registered population of the Commissioning Organisation ensuring delivery of provision in a variety of hard to reach and rural areas as specified by the commissioner.

The Adult Hearing Service will consist of:
• Hearing needs assessment
• Development of an Individual Management Plan (IMP)
• Provision and fitting of hearing aids in a suitable environment (quiet, confidential, privacy and dignity);
• Appropriate hearing rehabilitation e.g. patient education;
• Information on and signposting to any relevant communication/social support services;
• Follow-up appointment to assess whether needs have been met;
• Discharge from hearing assessment and fitting pathway;
• Aftercare service including advice, maintenance, repairs or replacement of hearing aids under warranty and review as required to meet the needs of the patient;
• Batteries, tips, domes, wax filters, tube and other consumables replacement service
• Help and support to the housebound

It is important to note the Contract does not guarantee any volume of activity or payment to a bidder.
