Description
NHS Ealing CCG is INVITING PROPOSALS with QUOTATIONS for the provision of time limited, expert resource to undertake a review of existing Continuing Healthcare Care packages of care and placements and Funded Nursing Care placements.
Interested providers will have a specialist understanding of the national framework for continuing healthcare. Interested providers must be able to mobilise quickly, ideally from within 1 week of agreeing the contract and complete the reviews within the contract term.
NHS Ealing CCG is looking for an external provider to work with the existing team to undertake a systematic review of patients (receiving both CHC and FNC) whose review is overdue by more than three months.
The Provider of this service will:
• Ensure that all patients who are receiving FNC have been reviewed within the past 12 months
• Ensure that all patients who are receiving CHC funded care have been reviewed within the past 12 months (or after 3 months when a new placement or package of care)
• When undertaking a CHC review ensure that their level of care is appropriate to their assessed level of need
• Using CareTrack, update patient records and ensure that future actions are identified, recorded and appropriately communicated.
• Where an FNC review indicates that a person may be eligible for full CHC to undertake a DST through a multi-disciplinary team approach.
• Liaise with family and other professionals around the timing of the reviews
The duration will be 3 months, with one optional extension of a further 1 month at the CCG's discretion and dependent upon success and resources.
It is intended that the new service will commence from April 2017 or as soon as possible. The total value of the initial 3 month contract is up to a maximum £100,000 contract with the possible 1 month extension included. The extension will permit the contract to continue to allow the provider to complete the activity if there were any delays and no further funding will be issued.
