Description
Sandwell and West Birmingham CCG are looking to commission a patient-centred Community Lymphoedema service. The aim is to commission a high quality, evidence-based, effective, equitable and accessible service for adults with Lymphoedema, regardless of aetiology, that ensures improved patient outcomes in relation to clinical needs, quality of life, health and well-being.
The service will provide specialist intervention, treatment and support to patients with primary or secondary Lymphoedema through clinic and domiciliary services. The service will aim to:
•Provide robust package of care for the specific needs of patients with primary or secondary Lymphoedema
•Encourage early identification, timely assessment and high quality treatment of Lymphoedema symptoms
•Ensure early recognition, awareness and knowledge of Lymphoedema, its prevention and management amongst all health care staff and professionals, and relevant patient groups
•Provide specialist advice and education to patients, carers and other appropriate health professionals regarding the management of Lymphoedema and associated wound management
•Comply with clinical guidance and good practice
•Provide advice and guidance with regards to the management and prevention of infection of Lymphoedema
The service will be able to meet multiple needs and will offer a choice of care options including clinic appointments and domiciliary care.
Currently there are two separate services which provide Lymphoedema care to Sandwell and West Birmingham patients. These services were historically commissioned by Sandwell PCT for Sandwell patients and Heart of Birmingham PCT for Birmingham patients.
The project will ensure a consistent service is commissioned across the CCG which delivers a high quality, evidence-based, effective, equitable and accessible service for adults with Lymphoedema, regardless of aetiology, that ensures improved patient outcomes in relation to clinical needs, quality of life, health and well-being.
The Contract period will be for 3 years with an option to extend for a further 2 years at the discretion of the CCG - so for clarity, the contract will start 01st April 2017 with the initial period ending 31st March 2020. There will be an option to extend at the discretion of the CCG to 31st March 2022.
