NWL CCG are looking for a supplier to create and build a Diabetes prevention and support community online, aimed at signposting people to NHS resources from the Know Diabetes and other Local Authority and Public Health websites to lower rates of preventable diabetes, and help people with diabetes to lower risk, gain successful treatment and make the lifestyle changes necessary to improve outcomes and quality of life.
