Description
The North Lincolnshire Health and Care system is seeking to block purchase a minimum of four adult mental health intermediate residential/supported living placements to support the discharge of people from Great Oaks Hospital. This approach supports the health and care system to ensure people have access to the appropriate level of support and are enabled to leave hospital as soon as they are medically fit to do so, continuing to receive care in the community setting.
