Description

NHS Wakefield CCG are seeking to commission additional providers to apply for this any qualified provider (AQP) tender process, subject to demonstrating compliance and competency against the specified criteria outlined in the specification, the successful provider(s) will deliver a Low Vision Aids Service to patients registered with a NHS Wakefield CCG GP Practice.

Provider(s) will be expected to sign the standard NHS contract, accepting all terms and conditions. The current contract for existing providers commenced on 1 May 2021. Existing providers need not apply, this process is for new providers and any accreditation will be for the remaining term of the contract up to 30 April 2024.

It should be noted that under the AQP model, there will be no guarantees of volume or payment in the contracts awarded.

The Provider(s) must ensure the services delivered are located within the boundaries of Wakefield District and accessible to patients registered with Wakefield GPs.

The overarching aims for this service are:
- Providing low vision assessments that are in accordance with individual need and expectation
- Provide aids that individuals can use to enhance their vision
- Intervening early in an individual's sight loss, by working with local health and social care services to identify individuals experiencing sight loss.
- Providing a continuous programme to improve the quality of the low vision service
- Ensuring the effective and sustainable use of resources

The overarching outcomes of the service are:
- Support for individual health and well-being
- Provide care in an accessible setting

Providers shall mitigate the practical, emotional, occupational or educational impacts of individuals sight loss.
