Description

The overarching aim of the Service is to help to reduce inappropriate admissions to hospital or premature admission to long term residential care, by providing a high-quality service delivered by staff that are appropriately qualified and trained to enable people with Dementia to live independently as long as possible.

Outcomes:

Outcome 1 - Engage and involve people in all aspects of Service delivery.

Outcome 2 - People who have accessed the Service report that staff were caring, compassionate and understanding.

Outcome 3 - People accessing the Service achieve positive outcomes. The Commissioner would want the Service to co-produce these outcomes measures with the people accessing the Service. However, the Commissioner would anticipate outcomes to be demonstrated in the following broad areas:

• Improved emotional wellbeing and mental health
• Improved overall physical health
• Decrease in isolation,
• Increase in confidence,
• Increase in self-esteem,
The method for capturing outcomes will be determined by the Service Provider.

Outcome 4 - The workforce is resilient and supported through the provision of supervision and continued professional development.

Outcome 5 - The Service is flexible and responsive, adapting to the individual needs of the person in terms of their circumstances i.e. strengths, level of risk, culture, ethnicity, language, age and disability.

The Contract will be for a term of two years with the opportunity to extend for one year and a further one year subject to mutual agreement between the successful Bidder and the Authority. The maximum term of Contract is consequently four years.

The available budget is £245,759 per annum. This figure includes all aspects of the service to be delivered, any bidder including caveats to this service may be considered as submitting a non-compliant bid. The value given in this notice is for a four year contract duration
