Description

Bradford Council and the three Bradford Clinical Commissioning Groups are seeking a provider who can develop and deliver an outcomes focused support service for Young Carers and their families, including undertaking assessment, developing personalised support plans and providing a range of support and interventions. In addition there will be an element of the provider undertaking Awareness Raising and Training of services on identifying and understanding the needs of Young Carers.
