Description
Oakmeadow is a 19 bed residential unit owned and operated by Halton Borough Council (HBC) in the vicinity of Widnes Town Centre. Admission to the unit is agreed following assessment by dedicated teams in the community and acute hospitals. The unit forms part of a range of Intermediate Care services within the borough which includes a 22 bed sub-acute unit, community multi-disciplinary team and a domiciliary care Reablement service.
This service specification is for the provision of medical services to the Intermediate Care beds at Oakmeadow Intermediate Care Unit, Peelhouse Lane, Widnes, Cheshire, WA8 6TJ (the \"Service\"). The provider of the medical service (service provider) will be responsible for the medical care of patients throughout the duration of their admission. All other aspects of service provision will be delivered by HBC with multi-disciplinary team in-reach support provided by the Rapid Access and Rehabilitation Service (a combined team of professionals from HBC, Bridgewater Community Healthcare NHS Foundation Trust and Warrington and Halton Hospitals NHS Foundation Trust)
The Intermediate Care beds provide care, treatment and rehabilitation for a range of potential patient groups. Pathways for admission will include:
continuation / instigation of re-ablement and rehabilitative care;
management of sub-acute exacerbation of chronic conditions;
treatment and care for single diagnosis conditions requiring a level of medical care that exceeds general medical services but does not warrant an acute hospital or sub-acute Intermediate Care Unit admission.
The Service will cover 19 permanent beds although this number may be subject to increase at times of capacity pressures in the system. The required medical provision for an increase in bed capacity and subsequent re-numeration will be negotiated between the commissioners and service provider. This will be monitored through the 3 monthly contract review meetings.
