Description
The service will cover all areas of Sheffield for the period 1st July 2020 - 31st March 2025 (4 years and 9 months).
We are looking for a creative and innovative Provider who will develop a high quality Young Onset Dementia 'Hub' to meet the needs of people with young onset dementia and their family carers.
The 'Hub' is not expected to be a physical building but is expected to serve people from across Sheffield so would offer activities utilising different existing locations across the city. The Provider may change the name of the service from 'Hub' with approval from the Commissioner.
