Description

NHS Vale of York CCG is seeking providers to deliver a Level 4 Anticoagulation Warfarin Monitoring and Management Service for the following practices:
• East Parade Medical Practice, York
• Beech Tree Surgery, Selby
• Escrick Surgery, Escrick
• Sherburn Group Practice, Sherburn in Elmet
• Tadcaster Medical Centre, Tadcaster

The Anticoagulation Warfarin Monitoring and Management Service in the NHS Vale of York CCG area was, in the main, historically provided by York Teaching Hospitals NHS Foundation Trust through a traditional phlebotomy-based service. The CCG is now transferring this service from secondary care into the community, using a near patient testing model.

NHS Vale of York CCG are looking for providers to provide on an equitable basis, a safe and effective initiation, stabilisation, monitoring, dosing and prescribing for a community-based anticoagulation service (warfarin monitoring and management). The community provider will be the first line treatment service for anticoagulation therapy and any patients who have been under the management of York Teaching Hospital NHS Foundation Trust anticoagulation service will be transferred to the community provider for continued management.

Currently there are circa 4000 patients across the NHS Vale of York CCG footprint who utilise the Anticoagulation Monitoring and Management Service and the service is predominantly provided in primary care apart from the 5 practices listed below. However, if any existing primary care providers opt out of providing this service in the future, we reserve the right (subject to the agreement of all parties) to potentially add additional practices at any point throughout the duration of this contract

NHS Vale of York CCG wish to establish a single Provider contract for the provision of the Anticoagulation Warfarin Monitoring and Management Service for the above 5 practices

Providers must use INRstar as the appropriate computerised decision support software (CDSS), which requires a secure NHS connection. All providers are expected to purchase the INRstar software licences and all hardware required to provide a point of care anticoagulation testing and dosing service.

The contract is due to commence on 1 January 2019 and the new provider must transfer 100% of all patients from York Teaching Hospital NHS Foundation Trust to their service, no later than 31 January 2019.
