Description
In partnership with Southport&Formby CCG & South Sefton CCG, NHSE (Cheshire&Merseyside) are looking to commission primary care medical services & management at Hightown Village Surgery&Freshfield Surgery for a period of 3years with an option of a further 2years.The delivery model should be innovative & in line with the principles of the General Practice Forward View.The procurement process will allow potential providers to bid for the two APMS contracts, respectively or collectively;Freshfield Village Surgery (Lot 1) & Hightown Village Surgery (Lot 2).The financial envelope is GMS rates,excluding Q&OF, NHSE DES & CCG commissioned services.Service requirements are to provide full time clinical services* Mon-Fri, in core hours from 8am to 6.30pm, so that patient needs are met in line with the GMS service specification, evaluation criteria and weighting, FMT and FMH *Definition of full time clinical services-patients seen in core hours by appropriate clinicians.
