Description
Hull City Council (HCC) is seeking suitably qualified and experienced providers to deliver two services, which aim to engage Hull employers (and their employees) in healthy workplace activities and increase the uptake and quality of NHS Health Checks across Hull (specifically to support an improvement of the uptake of NHS Health Checks among high risk and vulnerable groups). The delivery of NHS Health Checks by GPs and Pharmacies is subject to a separate procurement.
The contract will commence on 1 January 2017 and will run for 15 months, ending 31 March 2018, to coincide with the end of the 5 Year Cycle of the NHS health check programme.
The contract will be divided into the following lots;
• A Workplace Health and Well-being Service, which will engage local employers to improve the health of working age people in Hull. The service will assess workplace health needs; implement any appropriate intervention identified through this assessment (including delivery of NHS Health Checks) and support the employer in the development and implementation of a workplace health policy.
• A Community Outreach Service, which will target known health inequalities in communities with respect to identifying individuals at an earlier stage of vascular change. This service will deliver NHS Health Check assessments in community settings for people who are registered with their GP Practice but may not attend.
In addition to delivering these services, we are seeking a provider who is able to deliver social value benefits for the City.
