Description

Coventry City Council are seeking to commission a service to support adults who are at greatest risk of poor health including those who display multiple lifestyle behaviours like obesity, low levels of physical activity and poor diet, smoking as well as other at risk groups like people with mental health conditions. This integrated service will operate universal and targeted interventions and will deliver the NHS Health Check programme in Coventry.
