Description
NHS NW London ICB requires the provision of Hospital to Home Services in Kensington and Chelsea, Westminster and Hammersmith & Fulham starting 1 April 2023.
The aim of the service is to help people safely discharged home where they do not have a carer/ someone (e.g. family) who can look after them & they do not fall under statutory services remit but require support. This is generally for patients at risk of re-admission who would benefit from support at home. Patients will need practical & emotional to help them settle at home & reduce the risk of them being readmitted into hospital.
The service is currently being delivered by British Red Cross. The future service is required for a period of up to 2 years (1 year + extension option of 1 year), from 1 April 2023. If British Red Cross is the only capable provider, the ICB intends to award a further contract for up to 24 months from 1 April 2023 (estimated value £409,766, NHS Standard contract (shorter-form).
Service users would include residents registered with a Kensington and Chelsea, Westminster and Hammersmith & Fulham GP, over 60 years of age, deemed medically optimised, at risk of re-admission & who would benefit from extra support at home.
The provider must:
-Accept 87-93 referrals per borough per quarter, support 348-372 vulnerable older people per borough annually
-Provide 4 home visits (unless preference by phone)
-Support patients for 4 weeks
-Complete initial visit within 48 hours of discharge
-Ensure that 70% of service users are not re-admitted within 28 days of discharge from hospital
-Report health & well-being improvements of service users & report on bed days saved in line
-Make referral to statutory services where required
-Have strong relationships with Imperial & ChelWest
Support offered:
-Shopping/food in fridge
-Preparing meals/ drinks
-Basic housing issues support (such as hoarding & light essential cleaning)
-Topping-up gas & electricity meters
-Signposting to voluntary & community services
-Providing non-specialist support, inclusive of homeless patients, by helping them to access relevant services
-Providing & or signposting to income/benefits advice
-Providing emotional support settling at home
-Supporting isolation & lack of social networks
The ICB is committed to assess the needs of the service users & consider how it needs to secure the needs of the people who use the services, & improve the quality & efficiency in the provision of the services including through the services being provided in an integrated way.
Kensington and Chelsea, Westminster and Hammersmith & Fulham based capable providers may come forward by 17:00 on 13th April 2023 to engage with the ICB.
