Description

Southend on Sea Borough Council wishes to explore the potential of Technology Enabled Care (TEC) to address the above and its ability to deliver the following outcomes

More people taking personal responsibility for their own health and well-being, measured by;
reduced visits to GP where the underlying cause is not medical but loneliness and social isolation related
self-management of long term conditions at home where intervention by heath professional is triggered by exception through alerts- again reduces GP visit and A&E admissions- specific emphasis here on respiratory conditions and addressing deliver the community Respiratory QIPP
reduced A&E admissions underlying long term conditions and falls
reduced average length of stay in a residential care home as a result of later entry through extended independent living

The Council in partnership with South Essex Homes and the Southend CCG is also looking to pilot the Assistive Technology Enabled care in a 96 independent living residential block within the town. The proposed is envisaged to be piloted as a 'Living Laboratory'' subject to residents' agreement to participate in the extended use of TEC to address the issue of Loneliness, Self-management of long term condition and Falls.
