Description

This pilot will involve establishing volunteer support to LD and PD Service Users within the New Forest and Gosport district areas.

The service will take referrals of Service Users from the Council's LD and PD operational teams, and will recruit, train, match, and support volunteers to work with the Service User.

The voluntary organisations will be asked to deliver a service model which is client-led, person-centred, and builds on the strengths and assets of individuals.

Individuals will be supported to achieve the following:
· Reduced social isolation
· Improved physical wellbeing
· Improved emotional wellbeing
· Improved independence
· Improved community engagement
