Description

Barnsley Council wishes to commission a health and wellbeing service(s) in the community which will promote healthy lifestyles, reduce isolation, stimulate interest, and regain skills to promote and increase independence. This service is intended to adapt to the changing strengths and needs of each person who uses the service. The service will also provide respite for the carer. The service is primarily for older people needing any help, such as personal care or practical assistance, to live life as comfortably and independently as possible, because of age, illness, or disability. We want providers to work with us to help people to achieve the things that are important to them. These will be bespoke for each person. Currently the service is provided in three fixed site health and wellbeing centres (day centres), one of which is a dementia wellbeing centre.
