Description

The services to be provided are care quality commission (CQC) registered care and support for adults and/or children and young people living in South Tyneside with complex health needs who qualify for NHS Continuing Health Care (CHC), Children's Continuing Care (CC) and Section 117 aftercare, living in their own home.
CHC packages of care are solely funded by the NHS to meet adults' (aged 18 and over) care needs which arise
from disability, accident or illness. This includes care needs arising from physical, mental health and learning disabilities.
Children and young people's continuing Care (CC) packages are solely funded by the NHS to meet children's needs which are identified as complex.
Section 117 (of the Mental Health Act 1983) packages of care are jointly funded through the local authority and NHS Clinical Commissioning Group to meet needs which reduce the likelihood of readmission to mental health hospital by being both proactive and/or reactive.

The contract is broken down into 7 lots, dependent on client group, and bidders may apply for 1 or more lots.
