Description

Slough Borough Council (on behalf of Slough Borough Council, Bracknell Forest Council, Royal Borough of Windsor and Maidenhead and East Berkshire Clinical Commissioning Group) wishes to procure specialist support service for survivors of stroke after hospital discharge. This service will operate across East Berkshire, with an emphasis on promoting independence, increased awareness and robust referral pathways between partner agencies as well as preventing risk factors associated with stroke.
