Description
Derbyshire County Council Adult Care are inviting tenders from suitably qualified providers to deliver the Derbyshire Home from Hospital Support Service. This service will provide practical support for vulnerable people at the point of hospital discharge, or those in the community at risk of admission to hospital.
The service will support people who may not be eligible for social care / NHS support but whose needs are likely to escalate without any intervention. It will ensure vulnerable people are supported with initial practical support for up to 6 weeks to promote confidence and independence.
The service will support people through a range of interventions, including:
• Undertaking simple shopping
• Ensuring the home is warm and basic provisions are available
• Arranging for key safes to be fitted and lifeline systems to be provided
• Support with paying bills and making appointments
• Collecting medication prescriptions
• Support to maintain and improve social networks
• Support with advanced care planning beyond initial short term support
