Description

The Authority is looking for a Supplier for the provision of a number of providers and commissioners to support individual conversations for this identified group of people. Considerable progress has been made on the journey to deliver a reduction in the number of people with a learning disability and / or autism within an inpatient setting however, there continues to be a number of people whose length of stay is in excess of 5 years or where there are barriers are in place to understand a community pathway for an individual. As part of this continued work, the regional team wants to make sure that people with a long length of stay and / or barriers to a community pathway, have a clear person centred life plan in place to support their journey to a home in the community and to identify clear requirements such as care support and accommodation to achieve this.
