Description

We are looking to commission an evaluated training programme that will support an evidence-based, patient-centred alternative to centre-based cardiac rehabilitation. This will address the latest NICE guidance recommendation that patients with HF are offered 'a personalised, exercise-based CR programme in a format and setting (at home, in the community or in the hospital) that is easily accessible for the person"
The training course will focus on the key steps of successful facilitation of home based cardiac rehabilitation (HBCR) for heart failure patients and include sessions on psychology, behaviour change, physical activity and exercise, engaging the caregiver and further content/interaction designed to bring all of the components together. Secondary targets will include managing low mood and smoking cessation.

The successful provider will work with local rehabilitation teams to identify, engage and train cardiac healthcare professionals (usually a cardiac nurse or physiotherapist by background), as facilitators, enabling them to support patients and their carers in settings that suit their needs supported by accessible information and support.

The contract is for one calendar year. The proposed work must commence as soon as contracts have been signed and exchanged, ideally starting by 1st September 2021.

The provider will be expected to mobilise rapidly, taking steps to ensure that they are in a position to deliver the training and associated support as outlined, on time and to high quality standard.
