Description

Derbyshire Community Health Services NHS Foundation Trust (DCHS) is acting on behalf of the Derbyshire STP and aims to source a provider to deliver a development programme for front line staff (and those who support them) to increase their awareness and skills in coaching and communication so they can be applied in practice to improve our patient/service user ownership, engagement and management of their own health challenges. It also aims to increase awareness of Health Determinants to inform clinical practice.
