Description
Bolsover Partnership wishes to commission an Exercise Referral Scheme forming part of Bolsover Wellness Programme.
Bolsover Wellness adopts a social model of health, focusing on upstream approaches to deliver health promotion activities. It recognises the socio-economic pressure experienced by people within the district, and attempts to make the scheme accessible to all. Wellness is a partnership of both statutory and voluntary organisations with a specific link to Bolsover Partnership and its Health and Wellbeing Action group.
Supporting people to change their lifestyles to one which will benefit their health and reduce risks to health is a complex area of work. Key features of any successful approach are that they are:
• Patient centred
• Promote Health Behaviour Change - no single model for behaviour change should be used as it is acknowledged that there are multiple influences on individuals to support lifestyles behaviour change.
