Description
The aim of this service is to provide Service Users with advice and support to live a healthier lifestyle and to provide understanding of how changes in lifestyle behaviour can improve health, wellbeing, quality of life and longevity.
To be delivered in two models:
Model One - A community and outreach service delivered at premises designated by the Provider
Model Two - Will be delivered in the form of a home visit, and will be primarily for carers.
The service will be contracted as a fixed term, sole provider agreement, procured using the single stage open procedure. The contract initial term will be for two core years with an option to extend for up to two further years at the sole discretion of the East Riding of Yorkshire Council.
The estimated value includes all extension options. Submissions exceeding the available financial envelope will be excluded on the grounds that they are not affordable.
