Description

Staffordshire County Council is seeking to contract with Providers who can provide packages of care in thecommunity for children and young people with disabilities who meet the threshold for Domiciliary Care Support.
Providers must ensure that each child/young person receives appropriate person centred care, based on anassessment of their needs and preferences.
Providers must have an understanding, experience and regard to the following:
• Care Quality Commission Guidance
• Care Act 2014
• Mental Capacity Act 2005 and the Deprivation of Liberty Safeguards
• Children and Families Act 2014
The objective of this service is to provide flexible responsive Domiciliary Care in a timely, safe and dignified tosupport the needs of children, young people and their families within Staffordshire.
Those interested will be required to provide and evidence that they can deliver the following:
• Support for independent living
• Improved wellbeing for the young person and their family
• Unique and tailored package of support
• Sustained interaction within the family
• Partnership working and effective communication with the family and professionals
• Developing networking opportunities for young people and families
• Positive changes to the quality of people's lives
• A genuine desire to influence change for others
• Innovative and resourceful solutions to challenges
