Description

NHS Greater Huddersfield & North Kirklees CCGs are looking to appoint a provider for a Home Oxygen Assessment and Review Service. During the contract term this service may be extended to include NHS Calderdale CCG, however the finance value and activity for this area is not available at this time.

The aim of the service is to ensure a safe, high quality home oxygen assessment and review service for Kirklees home oxygen users. The objectives of the new service are to provide a systematic and integrated service that will be equitable across Kirklees and will:

1. Enable current national guidance and best practice to be implemented and adhered to
2. Improve the long term clinical outcomes for patients by being confident in self-managing their condition
3. to ensure a higher standard of clinical treatment and improved outcomes, through more effective and speedier diagnosis
4. To reduce/eliminate waste and poor quality care, and strengthen affordability and value, through targeting the service on those who will benefit from home oxygen
5. Reduce the risk to patient safety and harm
6. To provide easy access to assessment and follow up procedures carried out by appropriately qualified and trained healthcare professionals using appropriate diagnostic equipment.
7. Establish triangulated data collection across the local system
8. Maintain locally agreed pathways
9. Establish care plans
10. Reduce hospital admissions

For patients, this will mean regular timely assessments and reviews of Home Oxygen prescriptions and will mostly take place in patients homes although there may be community based services offered.

The service will operate seven days a week, 365 days per year and as minimum 08:00 - 20:00 hours.

High ethical standards, a duty of confidentiality and safeguarding responsibilities are mandatory along with being able to demonstrate GDPR and DPIA compliance.
