Description

The Children and Young People's (CYP) Transformation Programme has established a programme to understand what good integrated care looks like for CYP and develop a set of principles, guidance and key themes for systems.

Integration for CYP is about making sure their needs are met and that care is delivered in the best possible way. We also know that wellbeing of CYP is to do with more than health - education and wider local services play a key role. Health and care may be provided by various organisations including by NHS services, schools, local authority and social care professions and across different providers.

We are now seeking a support partner to work with the pilot sites in the early phase of the programme (for approx. 3 months) to further refine and scope the pilots, identify a baseline for each site and define what success looks like.

This work will form the foundations of a wider and longer-term evaluation of the programme
