Description
NHS Arden and Greater East Midlands Commissioning Support Unit (Arden & GEM CSU) on behalf of Southern Derbyshire Clinical Commissioning Group (SDCCG) and NHS England (North Midlands) is asking for expressions of interest from providers who would be interested in taking on a caretaker contract for a GP Surgery in Derby on a short term APMS Contract from the 1st April 2017 for a period of up to 12 months (6 months plus 6 month notice period).
Key points for consideration for interested providers are:-
• Must be able to demonstrate an ability to mobilise to service provision within a short time frame.
• Requires recruitment to GP positions as the two current partners are to resign and retire respectively.
During the caretaking arrangement, it is envisaged that the long term future of the service will be determined through public consultation and, depending on the outcome of the consultation; a possible re-procurement will be undertaken.
The practice has a current list size of approx. 6,430 patients and is provided from the following location:

The identified provider will have the option to continue to provide services from this premise and should note the premise is not part of the NHS estate.
The practice was inspected by the CQC in December 2015 and was given an overall rating of "Requires Improvement". They also received a rating of Requires Improvement for the following areas:

SDCCG and NHS England is looking for a modern, patient-centred model that is user friendly and culturally sensitive that considers all of the national and local public health targets.
The model will need to include provision of the following:
• Essential Services
• All Additional Services
• Dementia & Learning Disabilities
• Enhanced Services
• Opening hours and service delivery from 8am - 6.30pm Monday to Friday (except statutory bank and public holidays).
Due to the interim nature of this service, it is envisaged that the practice will operate under an open list and with a minimum income guarantee based on the current list size.

The Commissioners wish to invite suitably qualified and experienced providers to bid for the provision of this service. The Commissioners are asking that all providers are able to answer 'yes' to the following questions before bidding for this opportunity:
1. Are you able to ensure the service will commence from 1st April 2017?
2. Do you have staffing capacity to delivery this service from 1st April 2017?
3. Do you have the ability to register as a provider with the CQC at these premises by 1st April with CCG support if needed?
4. Do you have the ability to turn the practice around from one that requires improvement to a CQC rating of "good"?
