Description
Wigan Borough Clinical Commissioning Group (CCG) requires an external supplier to undertake an evaluation of the Community Link Worker service. The evaluation programme is expected to commence in January/ February 2016 and be delivered over a period of approximately 3-4 months. There is flexibility with regard to timescales, and we expect the supplier to work with the CCG and partners to determine the evaluation programme timetable and methodology.
The key objectives of the evaluation are as follows:
- To understand the impacts of the Community Link Worker intervention on services users and their families.
- To understand the impact of the Community Link Worker service on general practice, acute settings, adult social care and voluntary & community organisations.
- To assess the extent to which the Community Link Worker service is delivering the anticipated benefits identified through the original business case.
- To describe and review referral pathways, criteria and service user characteristics in order to inform and shape the service model.
