Description

The NHS Long Term Plan recommends implementation of high intensity models of stroke rehabilitation and highlights the need for ISDNs to bring people and organisations together to deliver the best possible care for their population. The 'Getting it Right First Time' (GIRFT) 2021 stroke report emphasized the need to address unwarranted variation in provision of community-based stroke rehabilitation.

The ICSS model includes provision of integrated and co-ordinated stroke early supported discharge (ESD) for all eligible patients, and community stroke or neuro rehabilitation for as long as patients need; provision of psychological and vocational rehabilitation, six month follow ups and close working with the voluntary sector to provide support services for stroke survivors and their families.

There are three pilot sites testing different models of implementation for the ICSS model as proposed by NHS England. Each pilot has a different focus specifically designed to implement the required changes within the context of their local stroke pathway. Each pilot has been chosen to explore the challenges and benefits of their approach in order to facilitate wider adoption of the service model.

These pilots are core to stroke programme activity and priorities in the long term plan. The models are expected to demonstrate significant savings allowing reinvestment in patient care.
