Description

NHS Medway Clinical Commissioning Group and Medway Council (the Contracting Authorities) are seeking to procure a high quality, effective and outcome focused service that will improve health and wellbeing outcomes for local people and offer good value for money. Commissioners want to pilot a new service model to support individuals living with long term health conditions to improve their skills, knowledge and confidence to manage issues impacting on their long term condition/s more effectively. The pilot will take place initially in one Local Care Team locality (Rochester).
