Description

Supported Housing Service for Young People with Complex Needs:
This service is a preventative homeless service, it is for vulnerable young people with complex needs aged between 16 and 25 with housing and support needs. The service is for the provision of safe, accommodation-based housing support services for young people who are deemed to be socially excluded this includes: - young people with complex needs, - young people with substance misuse needs, - young people with multiple needs, including mental health needs, - young people with challenging behaviour(s). It will provide housing related support services with the purpose of developing or sustaining an individual's capacity to maintain their accommodation and live independently in the community. These young people may be leaving institutional care such as hospital or prison, be street homeless, threatened with homelessness or living in accommodation that is not meeting their identified needs. The service will also be used as a 'step up' or 'step down' from other supported accommodation, where appropriate. Candidates should be able demonstrate your organisation's past experience and/or existing arrangements by giving examples of providing high-quality support for vulnerable young people. Candidates will also be required to provide an explanation of their organisation's capability and/or past experience of providing housing management in a supported housing or similar environment.
