Description

NHS Guildford and Waverley CCG is INVITING PROPOSALS for the provision of Adult Community Health Services.

NHS Guildford and Waverley Clinical Commissioning Group is offering a contract to the successful Bidder to provide Integrated Community Health Services for its adult population, grouped into the following themed areas:

• Proactive Care
• Intermediate Care
• Place Based Care

The Contracting Authority is seeking to award a contract to a Bidder that will work with the Contracting Authority and other key partners to deliver the highest quality 'out of hospital' services healthcare for its population. The overall aim of the procurement is to secure provision of integrated adult community health services to improve outcomes for the population with a specific focus on the following:

• 'No wrong door' - Single point of access for health and social care services;
• Improve access to integrated proactive community health and social care services;
• Support patients with complex needs to remain in their normal place of residence; and who will improve patient outcomes through integration to:

- Reduce complexity of services through integration across pathways of care
- Wrap services around Primary Care
- Build Multidisciplinary teams for people with complex needs, collaborating with Secondary, Mental Health and Social care and other services
- Provide Systems leadership, supporting these teams with specialist medical input and redesigned approaches to consultant services -particularly for older people and those with chronic conditions
- Develop innovative ways of working with the third sector
- Create services that offer an alternative to hospital stay

The Contract to be offered to the successful Bidder at the end of this Procurement will be for a duration of two years, which is intended to run from 3 April 2018 until 2 April 2020 with the possibility of one extension of up to seven years, subject to any restrictions placed nationally by NHS England or the Department of Health, i.e. a potential total contract duration of up to nine years.

It is intended that the new service will commence by latest 3 April 2018.
