Description
Portsmouth City Council (PCC) is in the process of considering our options for a Home Care Bridging Service. We are looking to streamline the service to work innovatively.
The purpose of this engagement is to share with you the key requirements for the service and ask you to comment on our requirements and confirm if you would be interested in providing this contract. Any information a provider shares could be used to inform the final specification for the new contract.
General Overview and Aims
o To provide a short-term domiciliary care solution as a 'bridge' prior to commissioning a long-term care package
o To facilitate timely discharge from hospital at the point that a person is medically optimised.
o To provide a 'bridge' for a person living in the community where a long-term care package may not be immediately available.
o Provide an opportunity for reablement and rightsizing as well as establishing long term needs.
Days/Hours of Operation
The service will be available from Monday to Sunday, 09:00 to 17:00 for referrals and 07:00 to 22:00, Monday-Sunday for provision of care.
However, this is purely a market consultation and is in no way an indication of the selection of your organisation or a guarantee of future work.
Following this consultation process, PCC will consider the appropriate route for this service.
